Repetition, Time & Past Roles
You may hear: “What time are we leaving?”
Pause and answer as though this is the first time. Then listen for the feeling or need behind the question.
Contact the person's health care professional if the repetition or confusion is new, suddenly much worse, or comes with signs of pain, illness, medication effects, or another rapid change.

Home, Place & Familiar People
You may hear: “I want to go home.”
Treat "home" as a possible need for safety, comfort, or familiarity rather than a fact to correct. First check whether the person is at immediate risk of leaving or is already missing.
If the person is trying to leave into danger, is missing, or you cannot keep either of you safe, follow the local emergency or missing-person plan now. Seek medical advice for new or suddenly worse disorientation.

Memory Conflicts, Accusations & Trust
You may hear: “Someone stole my purse.”
Pause before defending yourself. Treat the concern as real to the person while checking that an item is actually missing and that a genuine theft or abuse concern is not being dismissed.
Take a plausible report of theft, exploitation, neglect, or abuse seriously and follow local safeguarding or reporting procedures. Contact a health care professional if accusations are new, severe, or very distressing; get urgent help if there is a risk of harm.

What it is not

If someone is missing, there is immediate danger, violence or self-harm, or abuse may be occurring, follow the appropriate local emergency or safeguarding procedure now.
